Description
AI can give a medical student the answer in seconds. But can it make them a good clinician?
In Part 1 of this two-part conversation, Dr. Sal D’Allura joins the IDAAHub Podcast. He is a supervising physician at Atrium Health Wake Forest Baptist, board certified in family and osteopathic medicine, a medical educator, and a former residency director. He trained in the late 1980s, when there was no internet and you had to know the answer on rounds or know which textbook to find it in. Today, he watches students reach for Google and AI before they even try to answer.
His concern is simple: you don’t know what you don’t know. If you never learned the medicine, you can’t ask AI the right question, and you can’t tell when its answer is wrong. He believes that clinical gestalt, the sixth sense that tells you a patient is sick even when they say they’re fine, only comes from years of learning and seeing patients.
“You need to become the genius, and then use AI to supplement, not the other way around.”
In this episode, we cover:
Dr. Sal’s journey from family medicine and urgent care to supervising PAs and NPs
How medical training has changed from textbooks and libraries to Google and AI
Why AI only works if you know what to ask
The “index in the back of the book” analogy for using AI well
Clinical gestalt: the sixth sense AI can’t teach
Why passing the test isn’t the real test; the patient in the exam room is
Why setting AI guardrails has to start with the individual clinician
Necrotizing fasciitis, appendicitis, and diagnoses that take clinical suspicion
Why speed in emergency medicine leaves no room to “ask the computer”
Should medical schools ban ChatGPT? Dr. Sal’s view
Why he still tells students to pick up Harrison’s and read the book
When to use AI: as a supplement, not the source
Don’t miss Part 2, where Dr. Sal discusses AI ethics, AI scribes, and the patient whose life is saved when AI would have missed it.
